The Chromic Care Model (CCM) was developed to integrate a more patient-centered approach to medical symptoms. Often, when treating patients, acute symptoms were prioritized, while management of recurrent systems was neglected. The CCM model has helped promote the development and implementation of easier access to care for patients with behavioral problems. According to the authors of the article Mental Health in ACOs: Missed Opportunities and Low Hanging Fruit, CCM has proven to be an effective method for facilitating access to behavioral health. It has included mental health in the primary care sector, making it easier to screen patients and follow up. By enabling this in primary care, it has been possible to identify high-risk individuals more quickly and provide them with the most effective treatments. This over time could lead to a decline in the number of people using facilities such as those for behavioral health, reducing costs spent on mental health
